Mada je tretman za sada testiran samo na mi\u0161evima, stru\u010dnjaci smatraju da se radi o va\u017enom koraku u borbi s opasnim i rezistentnim patogenima.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Novi lijek, kao i mnogi drugi antibiotici koji se trenutno razvijaju, cilja dvostruku membranu koja okru\u017euje gram-negativne bakterije; poput Escherichije coli, koja mo\u017ee uzrokovati infekcije crijeva i krvi, i Klebsielle pneumoniae, odgovorne za infekcije plu\u0107a, mjehura i krvi. Gram-negativne i gram-pozitivne bakterije imaju razli\u010dite stani\u010dne stijenke, uzrokuju druga\u010dije infekcije te su protiv njih u\u010dinkovite razli\u010dite vrste antibiotika.<\/p>\n\n\n\n<p class=\"has-text-align-center has-luminous-vivid-orange-color has-text-color has-link-color wp-elements-1 wp-block-paragraph\"><strong>Druga\u010diji pristup<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Gram-negativne bakterije obavijene su za\u0161titnom kapsulom. Ova kapsula sprje\u010dava bijele krvne stanice (koje se bore protiv infekcija) da &#8220;progutaju&#8221; bakterije. Pod kapsulom, gram-negativne bakterije imaju vanjsku membranu koja ih \u0161titi od odre\u0111enih antibiotika, kao \u0161to je penicilin, ali im i omogu\u0107ava izlazak toksina i ulazak hranjivih tvari.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Znanstvenici ve\u0107 godinama poku\u0161avaju utjecati na vanjsku lipidnu membranu jer je ona klju\u010dna za funkcioniranje gram-negativnih bakterija. No stru\u010dnjaci iz \u0160vedske su primijenili druga\u010diji pristup. Tijekom testiranja su ciljali enzim LpxH. Njega koristi oko 70 posto gram-negativnih bakterija, a on poma\u017ee sintetizirati druge klju\u010dne komponente vanjske membrane &#8211; lipopolisaharide.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Znanstvenici su mi\u0161evima kojima je ubrizgana E. coli ili K. pneumoniae otporna na lijekove sat vremena nakon inficiranja dali dozu spojeva dizajniranih da inhibiraju LpxH. Rezultati su pokazali da je ovim pristupom mogu\u0107e izlije\u010diti infekcije krvotoka unutar samo \u010detiri sata, i to sa samo jednom dozom.<\/p>\n\n\n\n<p class=\"has-text-align-center has-luminous-vivid-orange-color has-text-color has-link-color wp-elements-2 wp-block-paragraph\"><strong>Gram-negativne bakterije &#8211; jedni od najopasnijih patogena<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Radi se o va\u017enom otkri\u0107u s obzirom na to da obje bakterije postaju otporne na antibiotike dostupne na tr\u017ei\u0161tu. Smrt od infekcija otpornih na antibiotike je u 2019. bio tre\u0107i vode\u0107i uzrok smrti na svijetu. Budu\u0107i da se ovi patogeni prilago\u0111avaju trenutnim tretmanima, potrebne su nove klase antibiotika kako bi se smanjio broj smrtnih slu\u010dajeva, pi\u0161e Science Alert.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Polovica antibiotika koji su danas dostupni na tr\u017ei\u0161tu su zapravo jednostavna varijacija lijekova otkrivenih prije gotovo jednog stolje\u0107a. Svjetska zdravstvena organizacija (WHO) je 2017. objavila popis najopasnijih patogena otpornih na lijekove, a na njegovom vrhu su gram-negativne bakterije rezistentne na naju\u010dinkovitije klase antibiotika \u0161irokog spektra.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">&#8220;Ovi rezultati su obe\u0107avaju\u0107i, no moramo obaviti jo\u0161 dosta posla prije nego \u0161to spojevi ove klase budu spremni za klini\u010dka ispitivanja&#8221;, navode znanstvenici sa Sveu\u010dili\u0161ta Uppsala. Klini\u010dka ispitivanja su testiranja na ljudima. Ispitivanje lijeka u laboratoriju ili na \u017eivotinjama je najranija faza razvoja tretmana.<\/p>\n\n\n\n<p class=\"has-text-align-center has-luminous-vivid-orange-color has-text-color has-link-color wp-elements-3 wp-block-paragraph\"><strong>Tri faze klini\u010dkih ispitivanja<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Svaki lijek prije izlaska na tr\u017ei\u0161te prolazi tri faze klini\u010dkih ispitivanja. U prvoj fazi sudjeluje manji broj ispitanika, obi\u010dno izme\u0111u 20 i 100 (ili rje\u0111e nekoliko stotina) te se njome utvr\u0111uje preliminarna procjena sigurnosti i u\u010dinkovitosti lijeka. U drugoj fazi se u\u010dinkovitost lijeka testira na ve\u0107em broju ispitanika, obi\u010dno izme\u0111u 50 i 300, te se analiziraju razli\u010diti na\u010dini primjene doza.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">U tre\u0107oj fazi sudjeluje najve\u0107i broj sudionika \u2013 od vi\u0161e stotina do vi\u0161e desetaka tisu\u0107a; ovisno o bolesti za koju se razvija lijek.
